Padma International Service Trip Application
Once you are ready to join a trip, complete the application below. Once we receive your application and $350 deposit, the Padma trip staff will review your application and contact you to confirm your spot on a trip, or to ask for more information. All trip members under the age of 18 will need their parent/legal guardian to complete this application with them. Please complete every section and

Select the trip you are interested in joining. Trip dates might change one or two days depending on student interest and airfare price. In order to join the optional surf camp in Bali, you must attend the first Bali trip June 4-18.

In the months prior to your student’s service trip and while we are abroad, Padma staff primarily communicates with parents via WhatsApp messaging app. It is available for free for both Apple and Android phones. Periodic messages will be sent via email and phone regarding trip preparation and planning meetings. It is important to let us know if any of your contact information changes. Please put the contact info for all parents/guardians who want to remain informed about the trip.

CODE OF CONDUCT
Our service trips require all members to represent themselves, their group, Padma Organization, and their country in appropriate ways. Doing otherwise will not only degrade reputations but can become dangerous and increase the risk of unfortunate incidents. Padma service trips are not extended parties and raucous behavior is not tolerated. Anytime you travel to new cultures there are particular
cultural mores and obligations regarding conduct. Some of these are common sense and others are unique to particular locations. The below list is designed to help trip members avoid potential problems and conflicts from arising during our trip, and to support constructive and positive group cohesion. All participants are required to abide by these guidelines for the duration of the trip. Failure to abide by this code of conduct can result in loss of activity privileges, fines, and expulsion from the trip at the student's/parent's expense.
Do not engage in any illegal activities during your trip. Ask trip leadership or simply consult google before your trip to learn about the laws of the country where you will be travelling. Consult trip leaders during your trip if you have any doubts about the legality or appropriateness of a given act. If you are not clear of the appropriateness of an action, avoid it. If you commit a crime while on a trip, there is no guarantee the Padma leadership staff can or will protect you from prosecution, fine, arrest, or incarceration. (Jails and prisons in the countries we travel to are not fun and the US Embassy can/will help very little if at all, once you are there.). If any trip member does engage in illegal or inappropriate behavior during a trip and posts photographs or writes about, their own or others, such behavior on any social media platform such as but not limited to: Snapchat, Twitter, Instagram, Facebook, WhatsApp, WeChat, email, group texts, etc., the member will be asked to leave the trip and fined $250. This pertains to trip related media posts made both during or after trips.
No illicit drug use is allowed on the trip under any circumstances.
Any prescription medications are to be brought from the US or purchased with the assistance of trip medical staff.
Participants are not to go into the homes of locals unless express permission is granted by trip leaders.
While visiting religious or cultural heritage sites, local customs such as the removal of shoes, and stopping idle conversation are to be observed. Also, be aware that inside some temples, photos are not allowed. Whenever there is a question of appropriate conduct, ask a trip leader for advice.
The location of any evening or nighttime plans are to be known and approved by trip leaders.
All tools and equipment used during project work are to be used as instructed.
Your attendance at all group meetings is required unless previous permission is granted by a trip leader.
Group members must inform trip leaders of any activities that include departing the project site and do not involve all group members, such as hiking in the mountains, ocean swims, village shopping etc.
All trip members including other students, trip leadership and chaperones are to be respected at all times. Fellow students are to be treated with kindness and any inter-personal issues are to be brought to trip leadership.
All trip members must be able to work as much as six hours at a time on any number of activities including: physical labor, fine motor skills, physical and mental dexterity. Students must be able to work independently with minimal direct supervision following initial instruction and guidance on a given task. Activities are performed in a range of temperatures and environments including water, sand, forest, dirt, mud and uneven landscapes. All trip members must be able to perform project activities independently and as productive members of the group.
Failure to follow these guidelines will result in disciplinary actions, including but not limited to: loss of activity privileges, assignment of additional duties such as bathroom cleaning, fines, and possible expulsion from the service trip at the discretion of Padma staff and trip chaperones, and at the expense of the student and their parent/guardian.

LIABILITY WAIVER
"I am aware that there are inherent risks and dangers to myself/my child in joining a service trip in south or southeast Asia. I understand that despite any and all efforts to mitigate dangers, a number of known and unknown risks still exist that may lead to the loss or destruction of personal property, and the injury to, or even death of myself/my child. I accept these potentials as unavoidable factors in joining this trip. I fully assume these risks, understanding that Padma Organization cannot be held responsible for any personal loss or disaster that I/my child may experience in connection with participation on this service trip.
I hereby agree to waive and release any and all claims and causes of action for damages or other relief that I may seek from Padma Organization, and against any of their affiliated or member entities, respective officers, directors, employees, agents, attorneys, or representatives, based on my/my child’s participation on a Padma Organization service trip. I acknowledge even if I/my child is a minor that I/they may be regarded as an adult under the laws of countries travelled to. I accept responsibility for my actions under those laws. I accept personal responsibility for my/my child’s actions outside the direction of Padma Organization personnel. I release Padma Organization of any responsibility for any actions in opposition of, or outside the guidance of Padma Organization staff and trip chaperones."

Conservation & Humanitarian Service Trips to Bali, Indonesia & Thailand require a significant amount of time in or near the ocean or other bodies of water. Some activities require Open Water or Advanced Open Water SCUBA certification.
Please rate your ability to swim (1-10) with a 1 meaning you are barely proficient to swim the length of a swimming pool, and 10 indicating an expert proficiency in all swim strokes and long-time experience on a swim team etc. Please rate your experience in the ocean on a scale of 1 – 10 with a 1 indicating little or no experience swimming in the ocean and a 10 indicating expert levels in ocean related activities such as surfing or scuba diving.

PADMA SERVICE TRIP PAYMENT AGREEMENT
The following document details the processes, requirements, and policies related to service trip tuition payments. If any part of this is not clear, or if you have any questions related to trip payments, please contact us before signing below. We encourage all trip members to apply for a scholarship as most applicants will receive some discount in trip tuition.
$350 Trip Deposit
: This payment secures your place on a trip. It is non-refundable and must be received by the trip signup deadline. Once your trip application and deposit are received your placement on a trip is confirmed. Following this you will receive a trip info packet and will be contacted by your trip leader.

Trip tuition may be paid all at once, or in three payments. If you choose to split your payments, the due date for each payment is:
February 15
(at least 1/3rd of trip tuition balance owed)
March 15
(at least 1/2 of trip tuition balance owed)
April 15
(entirety of remaining balance)

Late Fees
There is a $25 late fee applied to each late payment. An additional late fee of $200 is applied if any amount of trip tuition is unpaid after May 1, 2020. In the unlikely event that collections or legal services are required to collect trip payments, late fees, or fines such as those related to inappropriate behavior during or expulsion from a trip, all additional costs related to collecting payment will be charged to the student and their parent/guardian.
If you need to arrange a different payment schedule than is noted above, please contact us before the first tuition payment is due on February 15, 2020.
Cancellation Policy
If you decide to cancel your trip for any reason, or if the trip leadership decides to cancel the trip altogether based on safety issues or project disruption resulting from environmental issues such as natural disasters, or social unrest etc., trip members will receive a 50% refund of all paid trip tuition.
Trip Insurance
You may purchase trip insurance for $350 which provides for a 100% refund of paid trip tuition and trip deposit in the event you choose to cancel your trip for any reason, or if the trip is cancelled due to natural disaster or social unrest etc.
